Heart Transplant Team
You will encounter each member of our transplant team along your journey. Each person on the transplant team plays a vital role in providing transplant patients and their families expertise, knowledge, support, and understanding throughout the entire process.
Team members
- Advanced heart failure/transplant cardiologist
Our board certified physicians specialize in advanced heart failure and have expertise in advanced treatments including medications, transplant, and mechanical heart pumps.
- Transplant surgeons
The cardiothoracic surgeons will do the heart transplant surgery and care for you immediately after the surgery.
- Transplant coordinator
The nurse coordinator organizes all aspects of your care before and after transplant and will provide transplant education and care for all of your transplant needs. A coordinator is on call 24 hours a day for immediate needs.
- Pharmacists
Dedicated pharmacists have extensive experience with medications for transplant patients and will monitor your medications while you are in the hospital and at clinic visits.
- Dietitian
Professional dietitians will help manage your nutrition before and after transplantation. The dietitian can help you with any special dietary instructions or diets to manage coexisting medical complications such as diabetes, high blood pressure, or high cholesterol.
- Social workers
Our social workers help patients and their families deal with many issues that may arise, including lodging, transportation, finances, and legal issues.
- Other team members
Trained health professionals will evaluate you before transplantation and will make recommendations to the team. These team members include anesthesiologists, hematologists, infectious disease specialists, psychologists, and others.
